Travelling abroad soon? If you require travel advice or vaccinations, please contact the surgery at least 6 weeks before you travel.
To begin the process, please click here and complete the "Travel risk assessment" section. A member of staff may email you a questionnaire requesting additional information. A separate questionnaire must be completed for each person travelling, including family members.
Your completed questionnaire will be reviewed by our Practice Nurse, and one of the following outcomes will apply:
- No further action required
- Travel health advice required — the Practice Nurse will contact you
- Travel vaccinations required — you will be asked to book a nursing appointment
Please note that not all travel vaccinations are available on the NHS, and some vaccines may incur a private fee.
